For those of you in this category you can download any of the forms provided below, print, fill out and fax them to us to place an order. We have several forms available including a large all inclusive form or several specialty order forms with the most common items in that category.

We must collect sales tax in the state of Maryland so it will be added. Please make sure to pay sales tax to your local authorities. It is not worth the risk.

Orders are generally shipped by UPS ground however we may chose to ship by other methods depending on costs or delivery time. Other delivery methods can be arranged by calling DHG Specialties.
